DETAILS:
This role would suit an experienced PM with knowledge of LeDeR programme, and the work that is being carried out to implement the recommendations from the LeDeR reviews from individual localities.
NECESSARY EXPERIENCE
Knowledge of Trust, Primary Care and Community Services and how they function as individual services and as part of a systems pathways.
Able to write business case proposals, briefing papers and reports for SMG and provide quality reporting updates.
Able to engage clinical leaders and community groups in this work and identify individuals to champion LD and/or ASD transformation to reduce mortality rates.
Experienced in carrying out service audits to lead on the work for carrying out reasonable adjustments and implementing system changes and developing plans to carry out these recommendations.
Experienced in engaging hard to reach groups – BAME population and carry out network and workshop events to inform and promote access to services and respond to any barriers or issues that have led to dis-engagement.
A strong leader that is able to work on their own initiative and as part of a team and engage clinical team in this work.
This contract is initially for a 3-month period with strong possibility of extension up to 1year.
